In 2005 the University of Ottawa Health Services successfully applied to the Ministry of Health and Long Term Care to be one of the 170 newly created Family Health Teams across the province. The province's mandate is to improve accessibility to healthcare for more than 2.7 million Ontarians. In addition to the family physicians and other specialists working at UOHS, the FHT added a whole new team of healthcare professionals including nurse practitioners, clinical pharmacist, mental health counselors, chiropodist, dietician, psychologist, and psychiatrist. Total number of visits was 100,270.
In 2010 UOHS received a total of 170,731 visits.
A Family Health Team (FHT) is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you - the patient. Our team consists of doctors, nurses, nurse practitioners and other health care professionals who work collaboratively, each utilizing their experience and skills within the clinic to provide you with the very best and most up-to-date care, when you need it.
Through our Services and Programs, the UOHS FHT strives to:
- Promote optimal health and wellness by focusing on prevention, early identification, treatment, care and support for the most prevalent and chronic health conditions of our diverse patient community.
- Assess and treat chronic mental health conditions through consultation with our psychiatrist, and in partnership with allied resources within the Ottawa area.
- Use the latest and most effective technology to enhance our core primary care services and health promotion programs.
