YOU MUST PROVIDE PROOF OF INSURANCE WITH EVERY VISIT

All provincial health insurances are accepted and your card will cover most medical costs. Don't forget to bring your card with you when you come to see a physician!

Version Codes

For Ontario residents, if you have received a new card in the mail, it will have a version code, which is one or two letters added to your numerical OHIP number. All previous cards should be destroyed, as they are no longer valid.
If you are from Ontario and do not have your card, a Health Number Release Form should be completed, and proof of residency will be required. If your card is expired, you will be asked to pay in advance for services required, and you will later be reimbursed by your provincial plan. If you are an international or an exchange student, you must show a proof of subscription to the University Health Insurance Plan (UHIP). You may subscribe to UHIP at the International Office, 538 King Edward Avenue.

Annual charting fee for Quebec patients

Patients from Quebec that have Quebec provincial health insurance (RAMQ) have the advantage of using their card at the University of Ottawa Health Services, rather than having to pay at the time of their visit and then requesting reimbursement later from RAMQ.  Due to the additional costs involved in offering this service to our patients, the clinic instituted an annual charting fee for Quebec patients in 2005.  Please note that enrolled students of the University of Ottawa are exempt from this fee.
The fee is payable upon your first visit following September 1st of each year and is structured as follows:
$50 per year for individual patients who do have a family physician
$150 per year for families with a family physician (all family members must reside at the same address)
$20 per visit for patients with a family physician who decide to pay the annual fee
