UOHS Website - How to prepare for your appointment
Thank you for choosing the University of Ottawa Health Services Family Health Team for your health care needs. The following information is intended to help you make the most of your appointment with our healthcare professionals.
General Instructions – All Visits
- Where to find us: We have 3 separate clinic locations, i.e. 100 Marie Curie, 801 King Edward and 1 Nicholas. Please verify your appointment confirmation information to be sure you know which location you are going to. Directions to the clinic and where to find parking can be found on our website at www.uottawa.ca/health/
- Arrive on time: Please keep in mind that you will need time to park, get to the clinic where you have your appointment, check in with Reception and complete any required forms. Since we are located on the very busy University of Ottawa campus, parking can be challenging at peak times. OC Transpo comes right by the University of Ottawa and may be a good transportation alternative to consider.
- Medical Forms: Certain types of visit require specific documentation. Consult the visit descriptions below to determine if you will need to fill out any forms. To save time, complete the form, print it, and bring it with you to your appointment. Remember to bring your glasses!
- Cancel an appointment: To cancel your appointment now. Please note that there is a fee for appointments cancelled with less than 24 hours notice. Consult our cancellation policy for more information on cancellation fees for each service.
- Rebook an appointment: To rebook an appointment, call us at 613-564-3950.
Note: To e-mail an appointment request, indicate if the appointment is for a full physical or a regular follow up. You must also clearly state the dates and times you are NOT available. The earliest available appointment will be given.
If you are visiting UOHS for a specialized appointment please consult the list below to prepare for your appointment to help ensure best possible care during the visit.
Annual Physical Examination allows the doctor and patient to review your health history and physical exam and use the data to coordinate a personalized care plan for you.
Annual physical exams are very important as they provide you with optimal health care to help you live a longer and healthier life. The physical exam typically includes an update on your medical history, height, weight, blood pressure, a full physical examination and a review of immunization status and medications. For women, it may include a breast exam, a pelvic exam and a PAP test. For men, it may include a testicular exam, hernia exam, penis exam and rectal exam.

Remember to bring a complete list of medications you are currently taking, and any questions you have for your provider.
Civil Aviation Physical
Your appointment with a Civil Aviation Medical Examiner (CAME) will include a complete physical examination and eye exam. Bring your log book, and your glasses and contact lenses if you wear corrective lenses.
It is important to note that this visit is not covered by student fees or provincial health plans so you will have to pay for your visit. Consult the service fee page for the visit cost and payment options.

Diabetes Consult
Initial Visit: Bring a list of all the medications and any natural products and/or vitamins you are currently taking. In addition, bring your glucose readings for the previous 2 weeks.
Follow-Up Visit: Bring your glucose readings for the previous 2 weeks.

Immigration Canada Physical
Your physical will be done by a Panel Physician who has been authorized by Citizen and Immigration Canada to perform immigration physical examinations for Canada.
Bring your immigration documents, glasses or other corrective eyewear, passport and passport photos to your visit.
Note: Student fees and provincial health plans DO NOT cover Immigration Canada Physicals or immunizations. Consult the service fee page for UOHS fees and payment options.
There are no forms required for the Immigration Canada Physical appointment. Once you arrive to your appointment our reception staff will help you.
Physiotherapy
The initial physiotherapy assessment and treatment will be approximately 60 to 90 minutes in duration depending on the complexity of your injury. Most follow-up appointments are 30 to 60 minutes in length.
We require payment for all physiotherapy visits up-front. Patients with extended health care plans are recommended to submit their receipt to their private insurance company for reimbursement.
If you are late for your appointment, you may be required to reschedule for the next available date. Missed appointment fees may be applied. Consult the UOHS cancellation policy for more information.
Bring appropriate clothing for your physiotherapy assessments and follow-up appointments, for example:
- Shorts (for injuries involving body parts from the mid to lower back and below)
- Sleeveless top (for injuries involving body parts mid-back and above)
- Hair clips or elastics (to hold back hair if you are having neck, shoulder, back treated)
The physiotherapy centre does NOT do third party billing for:
- Motor Vehicle Accident cases
- Veterans Affairs
- WSIB
For claims involving these third party billing companies, contact a physiotherapy centre that is MVA, WSIB, or Veterans Affairs billing-approved. We can refer you to various clinics that offer these services.
There are no forms required for the Physiotherapy appointment. Once you arrive to your appointment our reception staff will help you.
Sports Medicine
In order to prepare for your visit:
Bring your x-rays (films or a CD), referral information, and any other documentation pertaining to your injury, i.e. previous reports
Bring appropriate clothing. This may include:
- Shorts (for injuries involving body parts from the mid to lower back and below)
- Sleeveless top (for injuries involving body parts mid-back and above)

Travel Clinic Your visit with a travel medicine specialist will include a health risk review of your travel itinerary, personalized travel health information, travel prescriptions and immunizations.

Note: Travel clinic services are not covered by student fees or provincial health plans. Consult the Uninsured Medical Services for fees and payment options
Vasectomy Appointments: Although a referral is preferred, patients without a family doctor may be given an appointment without a referral. Contact us at 613-564-3950 to book your appointment.
Two appointments are required for this procedure.

First visit – 10 minutes:
During this 10-minute consultation, your physician will explain the vasectomy procedure and you will be given an opportunity to ask questions.

Second visit – 30 minutes:
The surgery will take place at your second appointment. The procedure takes 10-15 minutes.
If you have a physical occupation, plan to be off work or on light duties for one week following the procedure.
Bring your valid OHIP or RAMQ card to both appointments. RAMQ patients will be required to pay $100.00 for additional costs not covered by your provincial health plan. The fee is payable at the first visit.
If you need to cancel or rebook an appointment, contact us at 613-564-3950. There is a $40.00 fee for a missed or cancelled consultation with less than 24 hours notice. The fee for a missed or cancelled surgery with less than one week's notice is $100.00. Consult our cancellation policy for more information.
