IMMUNIZATION

As a Health Care Professional, you have an obligation to protect your patients and yourself from infections that can be transmitted within health care practice settings. Immunization is an important tool in preventing the transmission of infections and assists in safeguarding your health during your education and beyond.

The University of Ottawa has mandated immunization/testing requirements for all students doing clinical placements. 

  • Required vaccination proof:
    • Tetanus/Diphtheria/Polio
    • Hepatitus B
    • Measles/Mumps/Rubella (MMR)
    • Influenza
  • Required serology:
    • Varicella
    • Measles/Mumps/Rubella (MMR)
    • Hepatitus
  • Required testing:
    • Tuberculin Skin Test (TST)

Procedure for students

The “Clinical Placement Requirements Record” form is available electronically or in hard copy at the Office of Risk Management (1 Nicholas St. Suite 840) or in Pembroke from Health & Community Studies Department (room 303, 315 Pembroke St. E.)

  • Depending on your program, you will need to complete the Clinical Placement Requirements Record form and submit it during the first year of admission. See Deadlines for program requirements. This initial record is valid throughout your degree program; however, you must submit proof of TST and influenza vaccination annually.
  • All sections of the Clinical Placement Requirements Record must be completed including specific dates for each immunization. *A comment such as “done in childhood” is not acceptable.
  • Plan for your Health Care Professional’s appointment several weeks in advance of the deadline: some vaccines require several doses and serology results can take up to four (4) weeks to obtain the results.
  • The student is responsible for all costs related to immunization.
  • Ask your Health Care Professional to complete and sign the "Clinical Placement Requirements Record".
  • Keep a copy of this record and all confirmations of immunization/immunity.
  • Submit your Clinical Placement Requirements Record to the appropriate address below:
