Laser Hazard Evaluation Data Sheet and Analysis Report
In order for the Office of Risk Management to provide you with an accurate hazard evaluation analysis report with exposure limits tailored to your typical operational lasers parameters in your laboratory, the following information is required for each laser:
- wavelength
- maximum anticipated exposure duration
- output average power, peak power or energy per pulse, if applicable
- pulse duration and pulse repetition frequency, if applicable
- beam diameter (based either on 1/e or 1/e2)
- beam divergence
- beam profile (if known)
- beam optics and beam path, which includes focal length of lenses, beam size at lens, beam profile at lens, spectral reflectivity of surroundings materials
These values should be as accurate as possible as small variations in the above parameters can have a dramatic impact on the hazard distance ranges. You can view a summary of the parameters needed in the Laser Hazard Evaluation Data Sheet whose link is provided below.

The outcome of these calculations are summarized in the Laser Hazard Evaluation Report – Exposure Limits (see link below) and may be viewed with your laboratory floor map.
