Complete Clinical Placement Requirements Record form:

• Consent to Release of Information form
• Proof of complete COVID-19 vaccination
• All vaccines, serological tests, and the 2-step tuberculin skin test (TST)
• One-Step TST OR Tuberculosis Signs and Symptoms Self-Declaration form (if previously tested positive):
  • Testing must be completed after May 31, 2026
  • If your 2-step TST is completed after May 31, 2026, the 1-step TST will not be required

Standard First Aid & CPR/AED Level C (for hospital placements):

• Valid for 1 year for hospitals, valid for 2 years for PHM 3505 course requirements.
• Certification must be obtained after May 31, 2026
• Cost may vary depending on the provider
  • Recommended options are:
    • St. John Ambulance (Standard/Intermediate First Aid Certification – CPR C & AED)
    • Canadian Red Cross (they have several training partners across the region) 
    • Heart & Stroke Foundation (Standard First Aid – Level C)

Crisis Management and De-escalation Techniques (CMDT) Training by Behaviour Management Systems (BMS):

• Valid for 1 year
• Dates to be confirmed by CPRM
• Cost: $226.83, payable directly to the provider
