Document authenticity and academic integrity

Any documentation submitted may be verified to confirm authenticity. The faculty reserves the right to request original documents from students at any time, to enable verification.

Falsification of documents is a serious offence and will trigger a formal investigation of academic misconduct. Such misconduct may result in disciplinary measures, including suspension or expulsion.

It is your responsibility to keep copies of all your documents, as we will not provide them. We are also not responsible for providing proof of immunization to other programs, clinical sites or future employers (including after graduation) and cannot help with this.

Deadlines	Requirements
November 15, 2027	Can be completed at ANY time. Some immunization requirements can take up to a year to complete, so it's best to start early. Student Placement: Immunization Requirements (immunization history and serology) Covid-19 Vaccination Documented evidence of full vaccination against COVID-19 is required. A full series consists of 2 or more doses if completed prior to October 2023 or 1 or more doses if completed after October 2023.
	To be completed only AFTER June 30th, 2027, to ensure the documents remain valid for the duration of the placement period. Annual Full Certification Course for CPR-HCP (Health Care Provider) or BLS (Basic Life Support)  N95 Mask Fit Testing 2-step Tuberculin Skin Test (TST) Proof of a baseline 2-step TST is required (two separate tests, ideally seven to 28 days apart) in addition to a yearly one-step TST. If you have a history of a positive TST, you must provide documented proof of the positive TST (date of injection and results in mm) along with the report of a chest x-ray completed after the positive TST and a completed Tuberculosis Signs and Symptoms Self-Declaration form.

Additional Requirement	Crisis Management Training Certificate is only valid for 1 year, so if your consolidation takes place in any Emergency, Traumatic Brain Injury or Mental Health department, you must submit a new certificate prior to starting your placement.
