Reporting a clinical injury or exposure

To report an accident or illness that occurred during your placement hours, you are required to complete the following steps:

1. IMMEDIATELY report the injury or exposure to:
   • Your clinical or placement supervisor
   • Your clinical coordinator
2. Report to the nearest emergency room for treatment (unless instructed otherwise by hospital Occupational Health).
3. Complete the uOttawa online Accident, Incident, Occupational Disease Form
