December 1, 2024	Student Placement Immunization Record (up-to-date vaccinations, serology, full series COVID-19 vaccination and 2-step TST) Seasonal Influenza Vaccination Annual Full Certification Course for CPR-HCP (Health Care Provider) or BLS (Basic Life Support) Valid for 12 months. Certificate must only be obtained after August 31, 2024, to ensure that it remains valid for the duration of the placement period. N95 Mask Fit Testing Valid for 2 years. Fit testing must only be obtained after August 31, 2024, to ensure that it remains valid for the duration of the placement period.
