Experiential Activities Form

This information is crucial for the uOGlobal team to confirm that you have met the requirements and are eligible for the uOGlobal recognition certification.
Pairing Activity

uOGlobal requires mandatory participation in a pairing activity. If you have not participated in any of the programs below, please select the "Other" box and tell us about your experience. For example, I did an informal match to help a new student on campus or hosted a new student in my home over the holidays.

Activity in a work context

Specify one workplace activity you participated in that you identified in your training plan. Please mention an activity with a minimum of 30 hours for the same organization. The activity may have taken place before your participation in the uOGlobal program as long as it was during your post-secondary education. The activity may be validated retroactively.

Note: If the activity is not included in the list of activities on the uOGlobal webpage and/or is not supervised by a uOttawa faculty or service, we require the contact information of the coordinator or manager. Submit this by choosing "Other" in the list. (Example: Volunteer, Local Church [Since 2018, coordinator: Michaël Beaulieu, @email, 613-555-5555]).

Activities in a course context

Specify the courses that you have attended, obtained a passing grade and that you have identified in your training plan.  The activities may have taken place before your participation in the uOGlobal certificate, but must be during your post-secondary studies.
