Hazardous materials technical services - special request


This form is to be used to request items such as: lab packs, laboratory decommissioning, laboratory decontamination, etc. For special transfers, kindly complete the Special Transfer Request Form available online or by contacting [email protected].


NOTE: The Office of the Chief Risk Officer may not cover the cost of the special request. If this is the case, the contact person will be advised prior to scheduling any services.

Kindly let us know when the services are requested to be completed. We will try our best to accommodate the timelines however, we cannot guarantee requested dates. We recommend planning ahead of time to secure services.

Hazardous material details

For lab packs, it is acceptable to send an excel list of chemicals to be disposed of, as a secondary attachment in the request.
