Should you have any Holds (Service Indicators) on your account, you will need to take the necessary action to satisfy them before you can enroll. If you have already submitted your Service Request, you must wait until the request is approved before enrolling.
Medical Absence Form
On the day of your absence: what to do
We recommend that you contact your professor in advance, or within 24 hours of your absence. In your email, be sure to indicate whether you intend to pursue a make-up opportunity to recover the grade.
Fill out the online form
Submit the Medical Absence Form along with supporting medical documentation within three (3) days of the missed graded activity for review. If you have any questions about completing the form, please contact the Wellness Office at [email protected].
After your form has been assessed
The Wellness Office will assess each submission and communicate with you should there be additional information required. If there is a valid medical reason for the absence, your course coordinator will be informed.

Important: Please note that this form is intended only for students who have missed graded activities. For missed examinations, including midterms and final exams, students must complete the Declaration of Absence from an Examination form. For further details, please refer to Academic Regulation A.8.6.

The appeal and exception process is made available to students who have encountered extenuating circumstances that have impacted their academic performance and subsequently resulted in Faculty decisions that could be reconsidered as a result of their unique situation.
Requests for an academic appeal or exception within the Faculty of Medicine are governed by the Policies and Regulations of the University of Ottawa.
